Simply Smiles provides mobile dental hygiene services within the legal scope of practice of a Registered Dental Hygienist in Alternative Practice (RDHAP). Services may include preventative and therapeutic dental hygiene care and are not a substitute for comprehensive dental treatment provided by a licensed dentist. Use of this website does not establish a provider-patient relationship.

All services require informed consent from the patient or a legally authorized representative, such as a legal guardian, conservator, or healthcare power of attorney, when applicable. Proof of legal authority may be required prior to treatment. Patients and/or authorized representatives are responsible for providing accurate and complete medical and dental history information.

Simple Smiles operates on a fee-for-service basis. Fees are determined on an individual basis depending on the patient's needs, treatment provided, and travel requirements. Payment is due at the time services are rendered unless prior arrangements have been made. Prices are subject to change without notice.

Appointments are scheduled in advance are subject to availability. I kindly request advance notice for cancellations or rescheduling. Late cancellations or missed appointments may result in a fee. Appointment policies will be discussed at the time of scheduling.

Simply Smiles provides care in non-traditional settings such as private homes, care facilities, or community locations. While all reasonable precautions are taken, services are provided based on the environment and access available at the time of the visit. Care is delivered in accordance with applicable laws, regulations, and professional standards.

Patient information collected through the website or during services is handled in accordance with applicable privacy laws, including HIPAA. Please refer to the Privacy Policy for details regarding the collection, use, and protection of personal and health information.
